Needed to register
Every child needs these on file before their first day.
- PDFEmergency FormContacts, pickup people and care details. Sign and date it, and mark “N/A” for anything that doesn’t apply.
- PDFHealth InventoryPart I is yours; the physical exam in Part II is completed by your child’s health care provider.
- PDFBlood Lead Testing CertificateMaryland asks for lead tests at 12 and 24 months of age.

Returning your forms
- Bring completed forms to the office, or send them with your registration.
- Your pediatrician’s office will also give you your child’s immunization record, which we need on file too.
